Elemental Mastery: 2 Elemental mastery allowed for beginers, you can train the rest later.
Villages:The village you want to be in, if you dont pick a village then you will become a missing ninja
Rank:the rank you wish to take. You take D rank,Gennin then you get mastery upto D-rank jutsus of your choosen elementary masteries,C rank,Chunnin-C rank jutsu mastery,B rank,Jounnin-B rank jutsu mastery,A rank,Special jounnin-A rank mastery,S rank,Kage-S rank jutsu mastery
Background info:not really neccessary but you can post it if you want to, its not going to affect anything
Character personality:again not neccessary but give it some lines.
RP Sample:post in a lot of info here if your going for a rank above D rank. You will have to give better RP sample to get bigger ranks, make use of characters emotions,surroundings,history,personality and yes use puntuation marks and good grammar.
